Functional Medicine Analysis
If the Functional Medicine Analysis is not received three days before the initial visit, the appointment will need to be rescheduled. In order to give exceptional care, the provider needs time to review your information.
Supplement Returns
Every human body is different and responds differently to each supplement. Just like any medication there can be reactions to supplements as well. If this happens and supplements are returned opened, the supplements will be accepted and 50% of the price will be refunded. We have no way to guarantee to other patients ythat the supplements were not damaged after the protective labeling has been removed, so we ask you to take your part of the responsibility. If they are returned unopened a full refund will be offered.
Also please be careful not to break the bottles because many of our products are housed in darkened glass bottles to be protective to their ingredients.
Late Policy
Sometimes things happen. Please give us a call if they do. If you are more than 15 minutes late for your appointment, it will need to be rescheduled. We value our time and that of all of our patients. It is also unfair to you as the patient to miss part of your full appointment. We want to provide the best care possible.
No Show Policy
There will be a $50 charge for no shows or less than 24 hour notice for missed visits. Patients are waiting for those visits and in functional medicine we are not in it for the money. Make sure to give us a call so that we have time to schedule another patient.
